Researchers from Optum are conducting a survey to explore quality of life in individuals who have been diagnosed with

  • Major depressive disorder,
  • Bipolar disorder,
  • Schizophrenia/Schizo-affective disorder.

Participation in this study will include asking your doctor to provide us with information regarding your health history. If you are found to be eligible, you will also be asked to fill out an online survey.

You may be eligible to participate in this study if you are:

  • An individual who is at least 18 years of age and is currently being treated for major depressive disorder, bipolar disorder, or schizophrenia/schizoaffective disorder.
  • Comfortable reading and communicating in English.
  • Willing to have your doctor provide information related to your health history
  • Willing to fill out a screening form and an online survey.

No medical treatment will be involved. Participation in this study is voluntary and consent to participate can be withdrawn at any time. Individuals selected to participate will be compensated $75 total in the form of a check once participation has been completed. This study is being conducted by Optum and is being supported by Teva Pharmaceuticals, Ltd.

All organizations (like Dbsa Tcsuon), in consideration for their time and effort, will be receiving a donation of $25.00 per patient as well.
